PAYROLL FORMS

Must be completed and in our office before a paycheck can be issued. We must have copies of the acceptable documents (i.e. State Drivers License, Social Security Card, Passport, ect.) with the I-9 form. Please check the I-9 form for a list of acceptable documents.
